Office Policy
We request 48 hours cancellation notice in the event a patient needs to cancel or reschedule an appointment. Patients who fail to cancel a scheduled appointment within 48 hours are subject to a $50 cancellation fee.
Patients are required to pay their copay at the time of their visit, and failure to do so may generate an additional $25 service fee.
Patients are responsible for obtaining a prior referral if their health plan mandates one, before seeing a specialist.
